My Path/Palliative Program

Palliative Partners has partnered with Riverside Family Physicians to provide a no-cost care management program for patients with advanced disease, like cancer, congestive heart failure or Parkinson’s. If you qualify, the program has a care team that is dedicated to delivering care to you in your home and providing supportive services to address your needs: physical, mental, social, emotional, spiritual.

How can the

program help you?

You’ll receive care from a team of Providers, led by a Doctor, who works alongside your regular Doctors to provide help when you need it:

  • Manage your pain and symptoms
  • Answer your questions about your disease, medicines and treatment options
  • Care Coordinator
  • Help you stay out of the emergency room and avoid being exposed to more health problems

There are counseling services when you’re feeling down. A social worker is on hand, as well as a chaplain, if needed.

These services are at no cost, and you can join or stop the program at any time.

MyDoc Anytime Program

Palliative Partners MyDoc Anytime is a patient care coordination program focused on optimizing and supporting the best quality of health for our patients. Eligible patients will have access to a care team that coordinates your care through prescription refills, appointment scheduling with your provider and the development of a personalized care plan.

How can the

program help you?

Every patient is assigned a nurse liaison who will work directly with you to coordinate care to ensure a healthier you. Your nurse is part of a team that will work in collaboration with your physician to insure you are getting the right type of care at the right place and the right time.
